Write down the exact decision
Which document was requested? By whom? For what process? Was an application rejected, delayed or never accepted? A clear description helps identify the responsible institution.
Keep a simple record
Save application dates, official receipts, reference numbers and written reasons. If information was given verbally, note the date, office and substance of the conversation. Do not publish personal identifiers.
Ask for the correction or review route

A referral is strongest when it explains why that institution may be responsible and what the person should take to the first contact.
